Telework forms is a VITA-built system that will provide workflow, notifications and dashboards (future release). VITA and the Department of Human Resource Management forged a joint partnership to automate the state telework form. The system will standardize business functions, collect valuable data for metrics, create process visibility and save agencies time.

The form is  available to all executive branch agencies. Please work with your agency's human resource team to coordinate adoption.

VITA has developed user guides and job aids to assist various audiences in their role with completing and/or approving the state telework form.
