Welcome to VITA's Online Customer Forms. It is VITA's goal that all forms be accepted electronically; however, for the time being there are still several that we request you submit as hardcopy with a signature as noted below.

Electronic submission: Upon completion of the entry of information, click "Submit". The next screen will provide an acknowledgement that your request has been accepted by our system. For your records an email copy of your submission will be sent to the email address you provided in the form.

Hardcopy submission: Hardcopy forms will have an Agency Authorization section at the bottom that includes instructions on where to send the form. Upon completion of the entry of information, click "Format to Print." The next screen will provide a presentation of the form with your data included. Print this off as you would any Web page, execute the authorization, and send the form to VITA. For your records an email copy of your submission will be sent to the email address you provided in the form.
