Virginia State Seal Public School Teacher Certification Form
For the Purchase of a Personal Computer
And / or Related Devices as per
Code of Virginia §2.2-2012


This Public School Teacher Certification Form is required to be completed by the Public School Teacher. Once the Form is completed, the Public School Teacher will receive an "Authorization Number". The Public School Teacher will submit the Authorization Number to the Computer Supplier for verification during the ordering process.
